Social Prescribers (Health and Wellbeing Advice)


Many things can affect your Health and Wellbeing such as anxiety, lack of motivation and isolation. A Social Prescriber may be able to help you.

Social Prescribers work together with patients to focus on what matters to them. They connect people to community groups, agencies and charities for support, advice and companionship.

What support could be offered?

Social Prescribers can help you –

  • If you are struggling with loneliness or social isolation
  • Achieve healthy lifestyle choices
  • Increase your mobility and balance-reducing trips and falls
  • Find appropriate exercise groups or classes
  • Access community groups or find local services
  • Find new hobbies and learn new skills including accessing music, arts, crafts or gardening groups
  • Meet others
  • Get back into or accessing work
  • Find volunteering opportunities
  • Access bereavement support
  • Gain support with other life changing events (i.e. moving house, retirement, births)

How it works

A Social Prescriber will arrange an appointment with you so that you can get to know and learn a bit more about each other.

The appointment will be confidential, non-judgmental and casual. Together you will discuss your interests and what is important to your life and wellbeing and which activities, services and/or charities may benefit you. You will set achievable goals and make plans together about your wellbeing.
The Social Prescriber will continue to support and encourage you along the way, including attending introductory groups so you don’t have to take the first steps on your own.

Who are our local Social Prescribers?

We have 4 Social Prescribers who work with Wyvern Health Partnership, the Primary Care Network which Lawn Medical Centre is part of.

They are not medically trained and therefore cannot give any advice regarding conditions, symptoms, diagnoses or medication.

NHS Social Prescribing is part of the NHS Long-Term Plan to highlight the benefits of personalised care across the health and care system.
Personalised care means people have options over the way their care is delivered based on what matters to them and their individual strengths and needs. Enabling patients to feel informed, have a voice and be connected to each other and their communities.
